Job summary

Agape Care Group in Oklahoma seeks a dedicated hospice medical social worker to conduct psychosocial evaluations and provide ongoing counseling to patients and families facing end-of-life care. You will collaborate with the care team to support emotional well-being, help with care planning, and guide families through challenging times with empathy and professional support.

Must have MSW and state licensure, with at least two years of clinical experience; reliable transportation is required.

Qualifications

• A heart to serve patients and families and a passion for providing the best possible care.
• Education: MS degree in social work from an accredited school of social work approved by the Council of Social Work Education.
• Licensure: Current state license as a social worker.
• Experience: 2+ years of clinical work experience, preferably in healthcare or hospice.
• Required: Reliable transportation and ability to lift up to 25 lbs.
