Peace Bridge Hospice in Cincinnati, OH is seeking a compassionate Hospice Social Worker (MSW/LSW/LISW) to provide psychosocial support, counseling, and care coordination to patients and families.

You will conduct assessments, facilitate end-of-life planning, educate families, and collaborate with nurses and physicians. A valid Ohio license is required and experience in hospice or palliative care is preferred.

Qualifications

• Master’s Degree in Social Work (MSW) is required.
• Active Ohio license (LSW or LISW) is required.
• Experience in hospice, palliative care, home health, or healthcare setting is preferred.
• Strong understanding of end-of-life care and grief support.
• Compassionate, patient-centered approach.
• Ability to work independently in a home-based environment or nursing home setting.
• Valid driver’s license and reliable transportation.

Position Overview

The Hospice Social Worker (MSW / LSW / LISW) is a key member of the interdisciplinary hospice team, providing psychosocial support, counseling, and care coordination for patients facing life-limiting illness.

This role focuses on improving quality of life by helping patients and families navigate end-of-life care, grief, and complex healthcare decisions.
