The Hospice Social Worker (MSW / LSW / LISW) is a key member of the interdisciplinary hospice team, providing psychosocial support, counseling, and care coordination for patients facing life-limiting illness.
This role focuses on improving quality of life by helping patients and families navigate end-of-life care, grief, and complex healthcare decisions.
Conduct comprehensive psychosocial assessments for hospice patients and families
Provide emotional support, grief counseling, and crisis intervention
Assist with end-of-life planning, advance directives, and goals-of-care discussions
Educate families on the hospice philosophy and available support services
Connect patients to community resources, financial assistance, and social services
Collaborate closely with nurses, physicians, chaplains, and hospice team members
Facilitate communication between patient, family, and care team
Provide bereavement and ongoing family support
Maintain compliant documentation and participate in care team meetings
Master’s Degree in Social Work (MSW required)
Active Ohio license (LSW or LISW required)
Experience in hospice, palliative care, home health, or healthcare setting preferred
Strong understanding of end-of-life care and grief support
Compassionate, patient-centered approach
Ability to work independently in a home-based environment or nursing home environment.
Valid driver’s license and reliable transportation
