Mobile Crisis Social Worker, Second Shift, Mobile Crisis Unit

UC Health is hiring a full-time Mobile Crisis Social Worker for the mobile crisis unit for the second shift at University of Cincinnati Medical Center.

The Mobile Crisis Social Worker responds to referrals from the interdisciplinary team, completes psychosocial assessments, develops and implements care plans addressing patients' needs, promotes continuity of care, and facilitates transitions to subsequent care levels. The role includes providing education, consultation, and acting as a liaison to the healthcare team regarding the emotional, psychological, social, financial, and developmental aspects of patient and family care.

Qualifications

Education and Experience Requirements:

  • Education: Master of Social Work from an accredited School of Social Work (required)
  • License and Certification: Licensed by the Ohio Social Work Board (LSW, LISW, LISW-S) (required)
  • Experience: 1-2 years (required), 3-5 years (preferred)

Hours: Full-time, 40 hours/week

Shift: Second Shift

  • Provide individual and family counseling related to crisis management, mental illness, adjustment, material assistance, healthcare decisions, and psychosocial barriers.
  • Advocate, mediate, and negotiate to create cohesive care plans.
  • Educate families about Advance Care Planning and assist with paperwork.
  • Facilitate discharge processes to various care settings.
  • Connect patients and families to community resources and arrange transportation.
  • Refer to disease-specific agencies, employment, financial, and support services.
  • Consult and train healthcare staff on psychosocial issues.
  • Participate in activities to improve customer service and patient satisfaction scores.
