UC Health is hiring a full-time Mobile Crisis Social Worker for the mobile crisis unit for the second shift at University of Cincinnati Medical Center.
The Mobile Crisis Social Worker responds to referrals from the interdisciplinary team, completes psychosocial assessments, develops and implements care plans addressing patients' needs, promotes continuity of care, and facilitates transitions to subsequent care levels. The role includes providing education, consultation, and acting as a liaison to the healthcare team regarding the emotional, psychological, social, financial, and developmental aspects of patient and family care.
