Job summary

Ohio’s Hospice is seeking a Medical Social Worker to provide psychosocial assessment, counseling, and care planning for hospice patients and families in Newark, OH. This role involves home visits, collaboration with an interdisciplinary team, and guiding families through advance directives and end‑of‑life planning.

The position offers weekday hours with shared weekend rotation and regular travel to patient homes and facilities in Newark and surrounding communities.

Qualifications

  • Bachelor’s degree in Social Work (BSW)
  • Current Ohio LSW or LISW license
  • Minimum 1 year of healthcare experience
  • Valid driver’s license
  • Reliable transportation and required automobile insurance
  • Excellent communication, organization, and documentation skills
  • Ability to work independently while collaborating effectively with an interdisciplinary team

Responsibilities

  • Assess psychosocial, emotional, financial, and practical needs of patients and families

Position: Medical Social Worker

As a Medical Social Worker, you will provide psychosocial assessment, counseling, care planning, advocacy, resource coordination, caregiver support, and end-of-life support to hospice patients and their families.

What You’ll Do

  • Participate as an active member of the interdisciplinary hospice team.
  • Collaborate with physicians, nurses, chaplains, hospice aides, volunteers, and other care professionals.
  • Complete accurate and timely documentation in the electronic medical record.
  • Provide compassionate, patient-centered care in homes, nursing facilities, and assisted living communities.

Monday–Friday | 8:30 a.m.–5:00 p.m.

  • Shared weekend and holiday rotation
  • Community-based position with regular travel
  • Visits to patient homes, nursing facilities, and assisted living communities
  • Service area includes Newark and surrounding communities
  • Reliable transportation required
  • Ability to travel in varying weather conditions
