This position is a full-time role, Monday - Friday, 8:00am - 4:30pm.

Description

The Social Worker (MSW) is responsible for non-clinical, biopsychosocial assessments and interventions for inpatient floors and outpatient emergency departments. Responsibilities include crisis intervention, patient/family support, high-risk screening, and referrals for financial or other resource needs. The MSW assists with post-acute care placements, especially for patients with behavioral health and resource needs, across units such as ICU, Trauma, Oncology, NICU, Transplant, and ED. The role involves responding to social work consults throughout the facility.

Key Responsibilities:

• Collaborate with healthcare team members to identify and address patient needs.
• Identify and resolve barriers to discharge and recovery, including financial, emotional, psychosocial, medical, and spiritual issues.
• Maintain knowledge of Medicare, Medicaid, and private payer regulations and managed-care concepts.
• Perform assessments and develop discharge plans for high-risk patients.
• Coordinate meetings between care teams and patients/families/caregivers, respecting their preferences and values.
• Provide emotional support and counseling for patients and families dealing with complex or emotionally charged issues, including end-of-life and terminal illness situations.

Qualifications:

• Master's Degree in Social Work required.
• Current licensure as a Registered Master of Social Work in Louisiana required.
• Minimum of 1+ years experience in an acute care setting.
• Licensed Clinical Social Worker (LCSW) and certification in Social Work preferred.
