Agape Care Group is seeking a hospice medical social worker to perform psychosocial evaluations and provide ongoing support to patients and families during end-of-life care. You will work in alignment with the plan of care to deliver compassionate emotional support when it's most needed.

Qualifikationen

• A heart to serve patients and families and a passion for providing the best possible care.
• Current state license as a social worker.
• 2+ years of clinical work experience, preferably in healthcare or hospice.
• Reliable transportation; ability to sit, stand, bend, and lift up to 25lbs.

Qualifications

• A heart to serve patients and families and a passion for providing the best possible care
• Education: MS degree in social work from an accredited school of social work approved by the Council of Social Work Education
• Licensure: Current state license as a social worker
• Experience: 2+ years of clinical work experience, preferably in healthcare or hospice
• Required: Reliable transportation. Ability to sit, stand, bend, move intermittently and lift at least 25lbs and bear the weight of an average adult effectively.
