Job summary

Trilogy Home Healthcare is seeking an MSW/LCSW to join our home health team for PRN visits in West Volusia County, Florida. You will provide compassionate care, work under clinical leadership, and help patients access community resources.

The role requires 1–2 years of experience, strong collaboration and communication, and the ability to exercise sound judgment and independent decision-making, with home care experience preferred.

What You Bring:

• Master's degree in Social Work (MSW) or Licensed as a Clinical Social Worker (LCSW) from an accredited school required for consideration
