Job summary

SC Evergreen Health Management Inc. in Rosemead, CA is hiring a Social Worker fluent in Mandarin, with Cantonese preferred, to support seniors and their families in our CBAS program.

You will collaborate with an interdisciplinary team to develop care plans, assess home environments, and provide counseling across a multilingual, compassionate setting. The role emphasizes enrollment, outreach, documentation, and liaison work with social service agencies.

Qualifications

• Master's Degree in Social Work or Bachelor's Degree in Social Work with relevant experience in aging/health/long-term care.
• Bilingual: Mandarin required, Cantonese preferred. Multilingual communication is essential for working with seniors and families.

Responsibilities

• Deliver excellent customer service as a full-time staff member during all operational hours.
• Welcome and orient visitors, families, and new participants to our staff, program, and facilities.
• Gather medical and financial information to establish participant eligibility and enroll participants.
• Develop individualized care plans with the interdisciplinary team and family input.
• Conduct home environment assessments prior to enrollment to evaluate living arrangements and accessibility.
• Provide counseling, education, and crisis intervention to participants and families.
• Act as liaison between participants/families and social service agencies, offering referrals.
• Facilitate support groups for participants, families, and caregivers.
• Organize outreach programs for participant recruitment and oversee intake and enrollment.
• Maintain accurate and timely documentation including progress notes and treatment plans.
• Assist in staff training and development, and participate in utilization review.

Key Responsibilities

Required Skills and Qualifications

Education and Experience

• Master's Degree in Social Work, Psychology, Gerontology, or Counseling from an accredited school with 1 year of social services experience in aging, health, or long-term care; OR


• Bachelor's Degree in Social Work from an accredited school with 2 years of social services experience.

Other Qualifications

• Bilingual: Mandarin required, Cantonese preferred.


• Experience in case management with elderly populations.


• Knowledge of the physical, mental, and social needs of older adults.


• Ability to work effectively in a multi-disciplinary team environment.


• Strong written communication skills and a collaborative, team-oriented mindset.
