Job summary

Cross Country Search is assisting a San Francisco client in finding a compassionate Social Worker Assistant to support older adults through care coordination, community resources, and interdisciplinary services.

This full-time role requires a Bachelor's in social work, current BLS/First Aid, and fluency in English and Cantonese and/or Mandarin. You will work with social workers, healthcare teams, families, and document in Epic while upholding HIPAA.

Qualifications

• Bachelor’s degree in social work or a related field.
• Current BLS and First Aid certification.

• Strong communication, organizational, and computer skills.

Responsibilities

• Assist participants with accessing community resources and social services while supporting social workers with case management and care coordination.
• Responsibilities include hospital discharges, transitions of care, home visits, phone support, family meetings, care planning, and documentation in Epic and other electronic systems.
• Collaborate with healthcare providers, community agencies, families, and the interdisciplinary care team while maintaining HIPAA and confidentiality standards.

Job Overview

Assist participants with accessing community resources and social services while supporting social workers with case management and care coordination.
