Job summary

Blaire House of Worcester is seeking an energetic Licensed Social Worker to uphold our high standards for elder care. Our 75-bed skilled nursing center specializes in dementia care, and we are family owned and operated.

You will develop social histories, provide counseling as a patient advocate, maintain progress notes in medical records, and craft discharge plans with community services; a licensed LSW or higher and at least one year of long-term care experience are required.

Qualifications

• Bachelor's or Master's degree in social work or related field.
• Must be a licensed social worker at LSW level or higher.
• At least one year direct social work experience in a long-term care facility.

Responsibilities

• Develop a comprehensive social history.
• Provide social intervention and counseling as patient advocate and liaison with community or financial issues.
• Maintain social service progress notes in patient medical records reflecting psycho-social and physical status, family interactions, and progress.
• Develop initial discharge plan and coordinate with available community services through discharge.

Job Description:

• Develop a comprehensive social history.
• Providing social intervention, such as counseling, as patient advocate, as liaison with the community, or regarding financial or emotional issues.
• Maintaining social service progress notes on patient medical records which reflect any changes in patient psycho-social or physical status, evaluation of family interactions, identification of new problem areas, and review of social plans, goals and progress made.
• Develop initial discharge plan and document in clinical record, revising until discharge with available community services.

Qualifications:

• EDUCATION: B.A. or M.A. degree in social work or related field.

Must be a licensed social worker at the LSW level of licensure or higher.

• TRAINING AND EXPERIENCE: At least one year direct social work experience in a

long term care facility. Must be a licensed social worker at the LSW level of licensure or

higher. Must have a genuine interest in working with geriatric patients and a sincere

concern for their welfare and happiness.
