Hope for Youth is seeking a dedicated Social Worker to provide critical support for youth in a group home setting. The role involves case management, crisis intervention, and collaboration with families and community partners. Candidates should have a valid LMSW or LCSW along with preferred experience in child services. Hope For Youth's Residential Services Department provides community based, trauma informed, youth and family driven care to adolescents and young adults in Nassau and Suffolk County. Serving youth from varying points of entry with the child welfare system, juvenile justice system, and runaway and homeless youth/young adults, the opportunities to alter the trajectory for youth in need are abundant. Youth are supported 24/7 by a team of multi-disciplinary professionals, paraprofessionals, and agency administration that consistently meets standards for accreditation and licensing regulations.
The Social Worker at the Seaford Boys Group Home must be a licensed social worker (LMSW or LCSW), providing informal clinical support and case management for up to 7 youth residing in the home. Maintaining a focus on the overall mission of the agency, and the Residential Department, the licensed social worker/Program Manager is supports and empowers our youth and their families to build resiliency and become self-sufficient to achieve their fullest potential. Work includes individual meetings, family and permanency team meetings, home visits, collaboration with community partners, and crisis intervention.
