Job summary

LCMC Health in Louisiana seeks a dedicated Social Worker to advocate for patients, assess psychosocial needs, and support discharge planning within a multidisciplinary team. You will assess barriers, coordinate resources, and provide compassionate counseling to patients and families facing complex medical situations.

The role emphasizes care coordination, crisis intervention, and collaboration with DCFS/EPS or other agencies to ensure patient-centered outcomes and timely discharges.

Qualifications

  • Licensed Master Social Worker through the Louisiana State Board of Social Work Examiners.
  • Experience conducting psychosocial assessments and discharge planning.
  • Strong communication and documentation skills.

Responsibilities

  • Conduct assessment of patient’s psychosocial needs through intensive interviewing of patient and family members, conferring with interdisciplinary team and reviewing medical records.
  • Evaluate coping skills, support systems, and barriers affecting illness, treatment and discharge plans.
  • Provide crises intervention and supportive counseling for individuals facing temporary or situational problems.
  • Identify and refer to community resources, arrange family meetings, and facilitate decisions around goals of care and plans of care.
  • Review caseload with manager to share findings, needs, barriers and progress to discharge.

Your Everyday

Conduct assessment of patient’s psychosocial needs through intensive interviewing of patient and family members, conferring with interdisciplinary team and reviewing medical records. Evaluate coping skills, cognitive and intellectual functioning, support systems, resources, otherfactors that could affect responses to illness, treatmentand discharge plan. Identifybarriers and plans for intervention to overcome or lessen barriers to achieve outcome asevidencedby treatment plan. Communicate findings, plan to interdisciplinary team anddocumentsassessment,planand interventions in medical records. Providecrisesintervention and supportive counseling for individuals experiencing a temporary or situational problem. Assess cases of suspectedelder, child, sexual or domestic abuse or neglect. Comply withrequiredreporting, according to state law and hospital policy. Refer patients/families toappropriate communityagencies for further intervention or counseling such as palliative or hospice services as needed andfacilitatefamily meetings to help with decision making when there is disagreement or lack of clarity around goals of care and plan of care. Facilitate interactions between staff and DCFS/EPS or other agencies. Identifypatients in assigned caseload with complex social and medical issues through case finding and referral process. Review caseload with manager to share findings, needs,barriersand progress to discharge. Evaluate financialassistanceneeds and eligibility and directs patients/family to appropriate community agencies which canassistin meeting financial needs, or providing food, shelter,transportationor other services. Maintain a working knowledge of payor reimbursement requirements for post hospitalservices and community resources. Demonstrate knowledge of legalrightsadvance directives and patient rights with ability to counsel/educate patients/familiesregardingpatient rights, decision making and formulating advance directives.
