Job Summary

Your role as a Social Worker involves serving as a key member of the multidisciplinary team, advocating for patients, assessing psychosocial needs, issuing discharge planning resources, and providing ongoing support to individuals with complex medical issues, including chronically ill and end‑of‑life patients.

Responsibilities

• Conduct a comprehensive assessment of patient psychosocial needs through intensive interviewing of patient, family members, interdisciplinary team collaboration, and medical record review.
• Evaluate coping skills, cognitive and intellectual functioning, support systems, resources, and other factors influencing illness responses and the treatment and discharge plan.
• Identify barriers and develop intervention plans to overcome or reduce barriers, ensuring outcomes are achieved per the treatment plan.
• Communicate findings and plans with the interdisciplinary team, documenting assessment, plan, and interventions in medical records.
• Provide crisis intervention and supportive counseling for individuals experiencing temporary or situational problems.
• Assess cases of suspected elder, child, sexual or domestic abuse or neglect and comply with required reporting in accordance with state law and hospital policy.
• Refer patients/families to appropriate community agencies for further intervention or counseling (including palliative or hospice services) and facilitate family meetings to aid decision‑making when there is disagreement or lack of clarity around goals of care.
• Facilitate interactions between staff and DCFS/EPS or other agencies.
• Identify patients in the assigned caseload with complex social and medical issues through case‑finding and referral processes.
• Review caseload with the manager to share findings, needs, barriers, and progress toward discharge.
• Evaluate financial assistance needs and eligibility, directing patients/families to community agencies that can assist with financial resources, food, shelter, transportation, or other services.
• Maintain current knowledge of payor reimbursement requirements for post‑hospital services and community resources.
• Demonstrate knowledge of legal rights, advance directives, and patient rights with the ability to counsel/educate patients/families about decision‑making and formulating advance directives.

Qualifications & Requirements

• Licensed Clinical Social Worker (LCSW) credentialed through the Louisiana State Board of Social Worker Examiners.
