Job summary

Ambercare Hospice is seeking a compassionate PRN Social Worker to join our refreshed team in Las Vegas, NM. This role supports patients and families with psychosocial needs, navigates benefits, and coordinates care within a trusted hospice program.

We offer relocation assistance for qualified candidates, flexible Monday–Friday scheduling, mileage reimbursement, and comprehensive benefits as part of a dedicated home health and hospice team.

Qualifications

• Social Work Degree.
• LMSW or LCSW licensure in NM.
• Minimum of one year’s experience in health care or hospice setting.
• Valid driver’s license and proof of insurance.

Responsibilities

• Performs psychosocial assessments for patients and families in the hospice program and documents needs in the plan of care.
• Provides exceptional service to patients, families, referral sources and teammates; supports colleagues as needed.
• Counsels patients and families about stress and coping strategies.
• Documents patient services accurately and timely to reflect the plan of care.
• Assists with applications for SSI, Hospice, Medicaid, and Medicaid Waiver; navigates community resources.
• Acts as liaison between patients, families and community agencies.

What You’ll Do:

• Performs the psychosocial assessment of patients and families/caregivers in the hospice program in a timely manner and ensures that identified needs are addressed in the patient’s plan of care. Provides an assessment in the patient’s identified residence and assistance when this is not safe, and another plan is required
• Demonstrates exceptional service to patients, families, referral sources, co-workers, and other customers. Assist team members as needed to ensure that this level of service is consistently provided
• Counsels patient and family/ caregivers and staff as needed in relationship to stress, and other identified coping difficulties
• Provides accurate and timely documentation of patient services to reflect the plan of care
• Provides assistance with applications for Supplemental Security Income, Hospice, Medicaid, and Medicaid Waiver. Obtains resources from community social, health, pharmaceutical and welfare agencies to assist in meeting patient/family needs
• Serves as liaison between patients and families/caregivers and community agencies

Qualifications:

• Social Work Degree
  • New Mexico: Master’s Degree in Social Work required.
• Current State Licensure in Social Work
  • New Mexico: LMSW or LCSW
• Minimum of one year’s experience in health care or hospice setting
• Valid driver’s license and proof of insurance is required
