Job summary

Ambercare Hospice in Las Vegas, NM is seeking a compassionate LPN/LVN to join our PRN team. You will provide expert clinical care and support to patients and families, prioritizing comfort, dignity, and quality of life in home-based settings.

This flexible PRN role offers autonomy to coordinate visits, manage caseloads, document care, and collaborate with an interdisciplinary team. Current state license, CPR, and driving eligibility required; home health experience preferred.

Qualifications

• Graduate of an accredited practical nurse or vocational nursing program with current license to practice in the state.
• 1 year medical/surgical experience preferred; home health or hospice experience is a plus.
• Current CPR certification.

Responsibilities

• Provides direct patient care as defined in State Nurse Practice Act.
• Implements current nursing practice following a comprehensive assessment and the plan of care.
• Documents patient services to reflect the plan of care in a timely manner.
• Educates patients and family/caregiver about diagnosis and self-care.
• Coordinates hospice services with the Interdisciplinary Group and reports needs.
• Manages pain and symptom control and evaluates patient response to treatments.

What You’ll Do:

• Provides direct patient care as defined in State Nurse Practice Act
• Implements current nursing practice following a comprehensive assessment and the plan of care
• Provide accurate and timely documentation of patient services to reflect the plan of care
• Assess and provide patient and family/caregiver education and information pertinent to diagnosis and self-plan of care
• Participates in coordination of hospice services, appropriately reporting the identified needs to the interdisciplinary group. Examples include hospice aide, OT, PT, MSW, Dietitian or Clinical Supervisor
• Provides appropriate pain/symptom management. Evaluates patient’s response to treatments/medications

Qualifications:

• Graduate of an accredited practical nurse or vocational nursing program with current license to practice in the state.
• Prior home health or hospice experience preferred w/ minimum of 1 year medical/surgical experience.
• Maintains a current CPR certification.
• Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.
