Corewell Health's Addiction Recovery Clinic in Niles seeks an MSW to provide individual counseling and supportive services, help patients achieve recovery goals, conduct crisis intervention, and coordinate care with community partners.
You will document assessments in the EMR, participate in care planning, and collaborate with physicians, nursing, and local agencies to reduce barriers to treatment and support long-term recovery.
Provides individual co-occurring counseling and supportive services to patients within the outpatient Addiction Recovery Clinic, helping individuals achieve and maintain recovery goals. Conducts crisis intervention, relapse prevention planning, and ongoing care management to support patient stability and long-term recovery. Collaborates with community partners, including the Berrien County Jail MAT/MOUD Program and the Berrien County Health Department, to coordinate services, strengthen continuity of care, and reduce barriers to treatment. Connects patients with internal and community resources to address basic needs and facilitate appropriate referrals. Works closely with the addiction medicine physicians, nursing, and community providers to promote whole-person care, prevent relapse, and improve recovery outcomes.
Conducts psychosocial assessments with patient and or family to assist with identified psychosocial needs or who may be identified to require an alternate site of care post discharge and executes a plan. Communicates with alternate sites of care and/or community agencies in an effective and timely manner, to best address the patient’s needs. Completes and documents the assessment and plan in the electronic medical record, accurately reflecting the patient’s current condition, situational factors, transition of care needs and psychosocial imperatives. Provides consultation and resources to members of the healthcare team. Responsible for maintaining relevant and current knowledge of community resources. Participates or assists in department, regional, or statewide stakeholder committees, projects etc., as assigned/requested. Develops and maintains current knowledge of federal and state regulations as they pertain to role.
