Job summary

Lawrence Rehabilitation and Healthcare Center is seeking a licensed social worker (LSW) to join our post-acute care team in a full-time role. You will evaluate psychosocial needs, coordinate care, advocate for residents, and support discharge transitions to ensure dignity and quality of life.

The LSW will work with an interdisciplinary team and maintain compliant documentation while upholding our core values of compassion and excellence.

Qualifications

• College degree in Social Work or related field preferred.
• Must have prior long-term care experience.
• Ability to make independent decisions with sound clinical and ethical judgment.
• Strong knowledge of social work practices and regulations governing post-acute care facilities.
• Ability to implement and interpret departmental programs, goals, policies, and procedures.
• Proficient in social services responsibilities, patient rights, safety, and sanitation standards.
• Demonstrates integrity and professionalism by adhering to the Marquis Code of Conduct and completing compliance training.

Responsibilities

• Build and maintain supportive relationships with patients and families to promote trust and engagement in care.

• Advocate for residents’ rights, emotional well-being, and access to services.
• Support discharge planning and transitions of care in coordination with the facility and external partners.
• Maintain accurate and timely documentation per facility policies and regulatory requirements.
• Ensure compliance with all laws, regulations, and standards governing social services in post-acute care settings.

Responsibilities for Licensed Social Worker (LSW)

• Evaluate patients (existing, new, and prospective) based on their psychosocial needs, strengths, and limitations.

• Support discharge planning and transitions of care in coordination with the facility and external partners.

• Ensure compliance with all applicable laws, regulations, and standards governing social services in post-acute care settings.
