Job summary

Lawrence Rehabilitation and Healthcare, a 120-bed sub-acute and long-term care facility in New Jersey, is hiring a Licensed Social Worker (LSW). This full-time opportunity offers $35-$40 per hour, all-inclusive, with a focus on counseling, care coordination, and advocacy to enhance residents' dignity and quality of life.

You will assess psychosocial needs, collaborate with medical teams, facilitate smooth transitions of care, and ensure compliant documentation.

Qualifications

• College degree in Social Work or related field preferred.
• Must have prior long-term care experience.
• Ability to make independent decisions with sound clinical and ethical judgment.
• Strong knowledge of social work practices, procedures, and applicable laws, regulations, and guidelines governing post-acute care facilities.
• Ability to implement and interpret departmental programs, goals, objectives, policies, and procedures.
• Proficient in social services responsibilities, patient rights, safety, and sanitation standards.
• Demonstrates integrity and professionalism by adhering to guidelines and completing compliance training.

Responsibilities

• Evaluate patients based on psychosocial needs, strengths, and limitations.
• Address patient concerns and goals while maintaining communication with patients, families, and case managers.
• Build and maintain strong relationships with patients and families to promote trust and engagement.
• Coordinate care and collaborate with interdisciplinary teams to support care planning.

• Advocate for residents' rights, emotional well-being, and access to services.
• Support discharge planning and transitions of care in coordination with the facility and partners.
• Maintain accurate documentation in accordance with policies and regulations.
• Ensure compliance with laws and standards governing social services in post-acute settings.

• Ensure compliance with all applicable laws, regulations, and standards governing social services in post-acute care settings.
