Job summary

Kenton-Nursing- seeks a Director of Social Services to oversee the social services department, ensuring quality emotional and social support for residents and families in accordance with regulations.

The role requires coordinating with medical staff, discharge planning, and development of care plans, with focus on ensuring compliance and high-quality resident care.

Qualifications

• Bachelor's Degree, LSW or MSW from an approved school of Social Work.
• Must possess current license/certification in the state of practice.
• Minimum of one year experience in long-term care preferred.

Responsibilities

• Keep up-to-date on federal and state regulations and professional standards; propose policy changes to the Executive Director.
• Implement policies for identifying medically related social and emotional needs of residents.
• Participate in discharge planning and development of social care plans and resident assessments.
• Maintain relationships with medical professionals and other health facilities and organizations.
• Conduct and document social services evaluations, identifying resident problems and needs.
• Provide or arrange social work services as indicated by resident/family needs.
• Keep residents' families informed of problems, needs, transfers, and changes in level of care.
• Meet with facility staff regarding resident issues.
• Ensure all documentation complies with applicable regulations and standards.
• Act in compliance with corporate, state, and federal standards; uphold honesty and integrity.
• Demonstrate a passion for caring through interactions with residents, families, and staff.
• Perform other duties as assigned.

As Director of Social Services, you are delegated the administrative authority, responsibility, and accountability necessary for carry out your assigned duties. You are responsible for providing services to respond to the emotional needs of the residents and their families. The primary purpose of your job position is to plan, organize, develop, and direct the overall operation of the Social Services department in accordance with current federal, state, and local standards, guidelines and regulations, our established policies and procedures, and ensure that the medically related emotional and social needs of the resident are met and maintained on an individual basis. Job Duties and Responsibilities

1. Keep up-to-date on current federal and state regulations, as well as professional standards, and make recommendations on changes in policies and procedures to the Executive Director.
2. Implement policies and procedures for the identification of medically related social and emotional needs of the resident.
3. Participate in discharge planning; development and implementation of social care plans; and resident assessments.
4. Maintain a quality working relationship with the medical profession and other health related facilities and organizations.

6. Provide/arrange for social work services as indicated by resident/family needs.
7. Keep the resident's family informed of resident problems, personal needs, transfers, and changes of level assignment.
8. Meet with the appropriate facility staff concerning resident issues.
9. Ensure all documentation complies with applicable regulations
10. Act in compliance with all corporate, state, federal, and other regulatory standards.
11. Conduct oneself with the highest degree of honesty and integrity in every interaction.
12. Demonstrate a passion for caring as evidenced by interactions with co-workers, residents, families, and visitors.

Must possess, as a minimum, a Bachelors Degree, LSW or MSW from an approved school of Social Work.
