Job summary

Elemental Management Group is seeking a Social Work leader to oversee the Social Service department in a skilled nursing facility in New York. You will provide consultation, direct education for Social Service and Nursing staff, and develop programs, policies, and procedures to support resident well-being.

You will conduct casework, counseling, and discharge planning, and drive performance improvement activities while collaborating on CQI initiatives to ensure high-quality care and regulatory

Qualifications

• Bachelor's degree in Social Work required.
• Current NY Clinical Social Work licensure required.
• Minimum 3 years of experience in social services and discharge planning.

Responsibilities
