HCA Healthcare Trident Hospital in Charleston, SC is seeking a Lead Social Worker to promote patient-centered care by evaluating psychosocial needs and coordinating discharge plans. You will mentor social work staff and collaborate with case management leadership to hire and guide team members.

The role requires a Master’s in Social Work, LMSW/LCSW license preferred, and prior hospital social work experience. Strong communication and crisis intervention skills are essential.

Job Summary And Qualifications

The Lead Social Worker is responsible for promoting patient-centered care by evaluating psychosocial needs, providing community service coordination for identified patient populations, and providing support and crisis intervention where indicated. The Social Worker acts as a liaison between the patient and community resources/post-acute care providers. The social worker creates and facilitates timely implementation of discharge plans and arranges post-acute interventions identified in the plan of care. The social worker facilitates patient movement across the continuum of care and identifies, addresses, and tracks barriers to patient throughput. In addition, the medical Social Worker team lead provides orientation and mentorship to new and existing staff, collaborates with the Director of Case Management in the hiring and interview process for new professional staff and manages a limited case load in addition to providing guidance and consultation to social work staff. Excellent analytical and team building skills as well as the ability to prioritize and work independently is required.

What qualifications you will need:

• A Master’s Degree in Social Work is required
• Licensure as a LMSW or a LCSW is preferred
• Prior experience as a Social Worker is required
• Prior experience in the acute care/hospital setting is strongly preferred
