Job summary

Qualifications

• Master’s Degree in Social Work required.
• Licensure as LMSW or LCSW preferred.
• Prior experience as a Social Worker required.
• Prior experience in acute care/hospital setting strongly preferred.

Responsibilities

• Promote patient-centered care by evaluating psychosocial needs.
• Coordinate community services and post-acute care for identified patients.
• Support discharge planning and arrange post-acute interventions.
• Mentor and guide social work staff; assist in hiring and interview process.
