Lifepoint Rehabilitation is seeking a Physical Therapist for an Acute Rehabilitation Unit in Ashland, KY. The role supports adult and geriatric patients within an inpatient rehab program, collaborating with therapists and nursing staff to maintain high care standards.

Responsibilities include assessing patients, developing plans of care, monitoring progress, and ensuring safety. BLS/CPR is required; inpatient rehab experience is preferred, with licensure in the state of Kentucky.

Responsibilities

  • Assumes responsibility for care of patients treated by Physical Therapist Assistants and therapy techs; may orient staff and students and cover supervisor duties in their absence.
  • Assess patients’ needs; initiate PT interventions; coordinate with the care team to maintain professional PT practice.
  • Observe and evaluate treatment effects; report progress and adjust plans with the physician if needed.
  • Perform assessments and regular reassessments; ensure timely QI scoring.
  • Manage patient pain to support function and participation in rehab; contact physician for interventions as needed.
  • Develop and update patient-specific plans of care for physical therapy.
  • Document patient progress and discharge barriers in team conferences; support patient safety and reduce care errors.
  • Perform all aspects of patient care in a setting focused on safety and quality.

Our team conducts comprehensive evaluations to determine each patient's unique needs, and then creates a multifaceted program using the latest rehabilitation therapies and advanced technologies to support them on their road to recovery. How you'll contribute

A Physical Therapist who excels in this role:

  • Assumes responsibility and accountability for the care of those patients being treated by Physical Therapist Assistants and Therapy techs. May be assigned to orient staff and students and assume the duties and responsibilities of the supervisor in the absence of that individual.
  • Assesses patients' needs; identifies and initiates appropriate physical therapy interventions; works cooperatively with other patient team personnel in maintaining standards for professional physical therapy practice and interdisciplinary teamwork.
  • Observes and evaluates treatment affect, accomplishment of short and long term goals. Recommends change to physician, if needed.
  • Performs an appropriate assessment on each patient and performs reassessments, as per policy. Performs accurate and timely QI scoring. The population is adult and geriatric patients requiring an adult medical rehabilitation program and services.
  • Able to assess patient pain interfering with optimal level of function of participation in rehabilitation; makes appropriate interventions to reduce pain; makes appropriate physician contact for intervention.
  • Formulates and updates patient specific plan of care for physical therapy
  • Reports patient progress and barriers to discharge in each patient team conference
  • Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors.
  • Other duties as assigned
What we're looking for

Applicants should possess the following qualifications and skills:

  • Education: Current licensure as a Physical Therapist in the state where the hospital is located.
  • Experience: Inpatient rehab experience preferred.
  • Additional Requirements: Current BLS/CPR card.
