AuthoraCare Collective in Greensboro (serving the Greensboro and Burlington campuses) seeks a Social Worker for Kids Path to assess and support the social, emotional, and practical needs of pediatric patients and their families across perinatal services, hospice, counseling, and home health.

The role requires a Master’s in social work, LCSW/LCSWA, at least one year in a mental health or medical setting, and ability to travel within the service area with a valid NC driver’s license.

• LCSW or LCSWA (or in the process of obtaining) required.
• One year of social work experience in a mental health or medical setting required.
• Must be able to travel to, enter, and function in the homes of patients/caregivers within the service area.
• Valid state-issued driver's license and automobile insurance required. Must have own transportation.

• Prepares psychosocial assessments.
• Develops and carries out individualized plans of care related to grief, loss, adjustment, care, and end-of-life planning for patients and families.
• Provides counseling including individual, family, or group counseling for pediatric patients and caregivers.
• Identifies appropriate resources to meet patient and caregiver needs.
• Serves as an advocate for patients and families.
• Actively participates with the interdisciplinary group (IDG) and communicates regulatory-compliant reports.
• Maintains relationships with community healthcare providers and faith communities.
• Completes required documentation according to licensure and agency standards.

Facilitates resolution of psychosocial/emotional conflicts which may interfere with a patient and family obtaining maximum benefit from Authoracare services. Provides educational programs to the community. Functions as a member of an interdisciplinary team of care. Position covers both Greensboro and Burlington campuses and serves the entire catchment area of Authoracare.

Requirements

Education/Qualifications

• Master's degree in social work
• LCSW or LCSWA (or in the process of obtaining) required.
• One year of experience in social work experience in a mental health or medical setting is required.
• Must be able to travel to, enter, and function in the homes of any patient/caregiver within the service area
• Other: Valid state-issued driver's license required. Must carry automobile liability insurance at limits required by agency. Must have own transportation

Key Responsibilities

Delivery of Care

• Prepares psychosocial assessments.
• Develops and carries out individualized plans of care related to the grief and loss, adjustment, care, and end of life planning needs of patients, families, and caregivers.
• Provides counseling including individual, family or group counseling for pediatric patients/caregivers/family.
• Identifies appropriate resources to meet patient/caregiver/family needs.
• Serves as an advocate for patients and families.

Interdisciplinary Group Process

• Actively participates with the interdisciplinary group (IDG).
• Works effectively with other IDG members with a focus on collaboration in problem-solving patient needs and working with the patients’ plan of care.
• Provides effective report to the IDG based on regulatory requirements for patient care planning evaluation.
• Maintains effective relationships and partnerships with community healthcare providers, agencies, and faith communities.

Documentation

• Completes required documentation according to licensure and certification requirements and agency standards including accreditation standards.
• Ensures that notes and patient records are complete, accurate, current, and clear.
