Job Summary

Serves as the social worker for the GUIDE (Guiding an Improved Dementia Experience) program, connecting patients and caregivers living with Alzheimer’s disease and related dementia (ADRD) with necessary medical and social resources. Provides social work services, complex case management, and care coordination in collaboration with primary care providers, neurologists, and community partners. Assesses needs, makes referrals, and contributes to developing comprehensive care plans to address ongoing health and mental health needs related to dementia.

Minimum Qualifications

• Licensed Clinical Social Worker (LCSW) in Illinois
• Master's degree in Social Work from an accredited CSWE school
• At least three years of social work experience, including two years in care of the targeted patient population in clinical settings, preferably in a hospital

Preferred Qualifications

• Accredited Case Manager (ACM-SW) certification

Responsibilities

• Complete timely documentation to meet licensure and third-party payor requirements
• Provide in-service training and attend educational seminars
• Participate in continuous process improvement activities

Knowledge, Skills & Abilities

• Competence in counseling and discharge planning
• Ability to recognize age-specific needs and behaviors

Physical Requirements and Working Conditions

• Ability to lift up to 35 pounds; use patient handling equipment for heavier lifts

Pay Range

$30.15 - $45.25
