Job summary

Lower Bucks Hospital in Bristol, PA, is seeking a Social Services II to provide emotional support, crisis intervention and counseling across pediatric to older adult patients, and to assist families coping with hospitalization, illness or trauma.

You will work as part of a multidisciplinary team, complete psychosocial assessments, help develop treatment plans, coordinate discharge planning, and document clinical records throughout the length of stay.

Qualifications

• Master's Degree in Social Work required.
• LSW or LCSW required.
• Minimum one year hospital social worker experience.
• Experience with diverse populations and complex social needs.

Lower Bucks Hospital is a community hospital, serving the Lower Bucks County region with medical, surgical, and emergency care. Located in historic Bristol, Pennsylvania, the Hospital is well-known for its knowledgeable medical staff, including cardiology, orthopedics, radiology, wound care, and general surgery. The hospital has more than 200 physicians on staff, as well as another 700 employees.

Responsibilities

SOCIAL SERVICES II: To provide emotional support, crisis intervention and counseling for Pediatrics, Adolescents, Adults, Older Adults and their significant others to cope with a multiplicity of social and emotional problems; information and referral; referrals for financial assistance and supportive services to patients and families with problems that are related to hospitalization, illness, disability or trauma.

To provide case management & milieu management, complete psychosocial assessments, assist in the development and re-evaluation of Treatment Plans and work as a member of a multidisciplinary team, and to coordinate discharge planning. Complete necessary documentation and maintain clinical record for each client during his/her length of stay.

Qualifications

Education and Work Experience

• Master's Degree in Social Work required.
• LSW or LCSW required.
• Minimum one (1) year hospital social worker experience with additional behavioral health experience.
• Experience in working with a diverse population with a wide range of problems (i.e., death and dying, maternal and child care, substance abuse, disability, chronic illness, etc.).
