Job summary

North Vista Hospital, a member of Prime Healthcare, is seeking a dedicated Social Worker to provide psychosocial support and counseling across age groups. The role emphasizes assessment, discharge planning, and collaboration with a multidisciplinary team in a hospital setting.

Requirements include a Master’s in Social Work, Nevada LMSW, and BLS within 90 days, with at least 1 year of inpatient/outpatient MH experience. This is a full-time, day-shift position in North Las Vegas, NV.

Qualifications

• Master’s Degree in Social Work from an accredited program.
• Active LMSW license in Nevada required.
• BLS certification within 90 days of hire.
• Minimum 1 year inpatient or outpatient mental health experience.
• Experience with diverse populations and complex psychosocial needs.

Responsibilities

• Provide emotional support, crisis intervention, and counseling for Pediatrics, Adolescents, Adults, and Older Adults.
• Offer information and referral services and coordinate referrals for financial assistance.
• Perform case management and milieu management within a multidisciplinary team.
• Complete psychosocial assessments and develop and reevaluate treatment plans.
• Coordinate discharge planning and maintain accurate clinical documentation.

Responsibilities

To provide emotional support, crisis intervention and counseling for Pediatrics, Adolescents, Adults, Older Adults, and their significant others to cope with a multiplicity of social and emotional problems; information and referral; referrals for financial assistance and supportive services to patients and families with problems that are related to hospitalization, illness, disability, or trauma. To provide case management & milieu management, complete psychosocial assessments, assist in the development and re-evaluation of Treatment Plans, work as a member of a multidisciplinary team, and to coordinate discharge planning. Complete necessary documentation and maintain clinical record for each client during his/her length of stay.

Required Qualifications:

1. Master’s Degree in Social Work field
2. Obtain BLS within 90 days of hire and keep current.
3. Must have an active Master Social Worker license in the State of Nevada (LMSW)
4. Minimum one (1) year Inpatient or Outpatient experience with emphasis in mental health care.
5. Experience in working with a diverse population with a wide range of problems (i.e., death and dying, maternal and childcare, substance abuse, disability, chronic illness, etc.).
