Job summary

• Experience with diverse populations and a wide range of problems (death and dying, maternal/child health, substance use, chronic illness, etc.).

Responsibilities

• Provide emotional support, crisis intervention and counseling to patients across pediatrics to older adults and families.
• Coordinate discharge planning as part of a multidisciplinary team; complete psychosocial assessments and contribute to Treatment Plans.
• Maintain clinical documentation and records for each client during hospitalization.
• Provide case management and milieu management as part of treatment and supportive services.

Responsibilities

To provide emotional support, crisis intervention and counseling for Pediatrics, Adolescents, Adults, Older Adults, and their significant others to cope with a multiplicity of social and emotional problems; information and referral; referrals for financial assistance and supportive services to patients and families with problems that are related to hospitalization, illness, disability, or trauma. To provide case management & milieu management, complete psychosocial assessments, assist in the development and re-evaluation of Treatment Plans, work as a member of a multidisciplinary team, and to coordinate discharge planning. Complete necessary documentation and maintain clinical record for each client during his/her length of stay.

Required Qualifications

• Master’s Degree in Social Work field


• Obtain BLS within 90 days of hire and keep current.


• Must have an active Master Social Worker license in the State of Nevada (LMSW)


• Minimum one (1) year Inpatient or Outpatient experience with emphasis in mental health care.


• Experience in working with a diverse population with a wide range of problems (i.e., death and dying, maternal and childcare, substance abuse, disability, chronic illness, etc.).
