Moore County seeks a Social Worker II for the Care Management Program (CMARC/CMHRP) to assess, plan, and follow up with at-risk children and high-risk pregnancies. The role includes counseling and coordinating with schools, foster families, and community partners to support habilitation plans and behavior management.
Qualified candidates hold a Master’s in a human services field or a Bachelor’s in social work (or related) and must maintain a valid North Carolina driver’s license.
This position works as a Social Worker II with the Care Management Program. The program serves the populations of at-risk patients through specific Care Management for At-Risk Children (CMARC) and Care Management for High-Risk Pregnancies (CMHRP). CMARC and CMHRP provide coordination of services and ensure care for clients. Work involves evaluating the client's situation and ability to deal with it, developing a social history, psychosocial assessment, service plan and/or treatment plan, and follow-up. Services may include basic counseling, support and reinforcement in areas such as: employment, vocational, medical, health, or behavioral issues. They may provide information to and confer with schools, natural parents, and foster parents on habilitation plans, behavior management, and other problem areas or needs.
This is light work requiring the exertion of up to 20 pounds of force occasionally, up to 10 pounds of force frequently, and a negligible amount of force constantly to move objects; work requires stooping, reaching, standing, walking, lifting, grasping, and repetitive motions; vocal communication is required for expressing or exchanging ideas by means of the spoken word and conveying detailed or important instructions to others accurately, loudly, or quickly; hearing is required to perceive information at normal spoken word levels and to receive detailed information through oral communications and/or to make fine distinctions in sound; visual acuity is required for preparing and analyzing written or computer data, visual inspection involving small defects and/or small parts, operation of machines, operation of motor vehicles or equipment, determining the accuracy and thoroughness of work, and observing general surroundings and activities; the worker is subject to inside and outside environmental conditions and atmospheric conditions.
