Job summary

Northwestern Medicine seeks a Social Worker II Flex to support high‑risk patients and families with complex psychosocial and economic needs. You will screen, assess, and coordinate discharge planning, delivering psychosocial interventions to promote safe transitions.

Collaborate with the care team, educate families about resources, document progress, and participate in department activities. Illinois licensure is required; MSW preferred; hours are 8:00 a.m.

Qualifications

• Masters Degree from an accredited School of Social Work or Bachelor's Degree in Social Work and three years of experience in medical setting preferred and enrolled in MSW program preferred.
• Licensure in Illinois. Licensed Social Worker or Licensed Clinical Social Worker (LCSW).

Responsibilities

• Provide discharge planning and psychosocial interventions to patients and families.
• Identify psychosocial and discharge planning needs of patients.
• Educate patients/families about community resources and involve them in intervention planning.
• Document progress per departmental policies.
• Collaborate with care teams to coordinate services and resources.
