Job summary

Beloit Health System is seeking a Wisconsin-licensed Social Worker/Mental Health Therapist to join the Counseling Care Team in a school-based setting. The role involves individual, group, and family therapy for youth and adolescents, with collaboration across school personnel and hospital partners.

The position requires a Master’s degree in Social Work or Counseling, appropriate licensure, and at least 3,000 post-graduate hours. Strong communication and EMR experience are highly valued.

Qualifications

• Master's degree in Social Work, Counseling and Guidance or related field.
• Wisconsin licensure is preferred (LPC, LCSW, LMFT) or minimal license LPC-IT/MSW-IT.
• Ideal candidate has 3,000+ post-graduate hours.
• Strong verbal and written communication skills.
• Ability to work independently and within a team.
• Experience with electronic medical records is a plus.

Responsibilities

• Provide individual, group, and family therapy to youth and adolescents.
• Perform psychosocial evaluations and develop treatment plans.
• Utilize DSM-5 standards for diagnosis and treatment planning.
• Lead crisis management within a multidisciplinary team.
• Coordinate with school personnel and build referral networks within the hospital and community.
• Participate in an on-call rotation (1–2 weeks per year).

The Counseling Care Center is seeking a Wisconsin-licensed, School-Based Therapist to work with youth and adolescents. The role involves individual, group and family therapy within the school Setting and collaborating with school personnel.

• Performs comprehensive mental health treatment to patients and families
• Utilizes DSM-V standards for diagnosing
• Completes psychosocial evaluations on all identified clients
• Completes treatment plans and treatment plan reviews, focusing on the improvement of mental health symptoms and emotional/personal functioning
• Provides competent and comprehensive treatment approaches to mental health diagnosis and emotional struggles
• Implements preventative strategies to mental and emotion disorders
• Takes a lead role in managing client's mental healthcare needs through case management and advocacy
• Provide treatment to diverse populations and ages, including individuals, families and groups
• Builds referral relationships within the Hospital setting and the treatment community, for referral and care networks
• Effectively manages crisis patient needs, facilitating care within the multidisciplinary team, in an empathetic and knowledgeable manner
• Participates in an on-call rotation (1-2 weeks per year)

Job Requirements

• Master's degree in Social Work, Counseling and Guidance or related degree from an accredited university
• Ideally Licensed as LPC, LCSW or LMFT in Wisconsin (Minimal License is LPC-IT or MSW-IT)
• Ideal candidate will have 3000 hours post-graduate experience
• Excellent verbal and written skills
• Must have the ability to work independently with minimal instruction and effectively within a team setting
