Job Summary

The Social Worker is responsible for facilitating care along a continuum through effective resource coordination to help patients achieve optimal health, access to care, and appropriate utilization of resources, while respecting the patient’s resources and right to self‑determination. The individual in this position assesses patients for transition needs, identifies those at risk for readmission, conducts complex psycho‑social assessment and intervention, and promotes timely throughput, safe discharge, and prevention of avoidable readmissions. The role integrates national standards for case management, includes transition management, care coordination, compliance with regulatory requirements, education of physicians, patients, families, and caregivers, and leads a population of patients by service line or team.

Responsibilities
  • Complex psycho‑social transition planning assessment, reassessment, and intervention
  • Assistance with adoption, abuse, and neglect cases, including assessment, intervention, and referral to appropriate agencies; care coordination
  • Implementation and oversight of transition plans, leading and facilitating multidisciplinary patient care conferences including Complex Case Review, and making appropriate referrals to other departments
  • Communicating with patients and families about the plan of care, collaborating with physicians, office staff, and ancillary departments, and ensuring patient education is completed to support post‑acute needs
  • Timely, complete, and concise documentation in the Case Management system and maintenance of accurate patient demographic and insurance information
  • Perform related duties as assigned
Qualifications
  • Required:
    • Education: Master's of Social Work
    • Licensure/Certification: MSW; must be currently licensed, certified, or registered to practice the profession in the state of practice; active license for covered state(s)
  • Preferred:
    • Experience: 2 years of acute hospital experience
    • Licensure/Certification: Accredited Case Manager (ACM)
Salary

$23.47 – $35.21 hourly (individual wages determined based on qualifications and experience)
