The Social Worker is responsible for facilitating care along a continuum through effective resource coordination to help patients achieve optimal health, access to care, and appropriate utilization of resources, while respecting the patient’s resources and right to self‑determination. The individual in this position assesses patients for transition needs, identifies those at risk for readmission, conducts complex psycho‑social assessment and intervention, and promotes timely throughput, safe discharge, and prevention of avoidable readmissions. The role integrates national standards for case management, includes transition management, care coordination, compliance with regulatory requirements, education of physicians, patients, families, and caregivers, and leads a population of patients by service line or team.
$23.47 – $35.21 hourly (individual wages determined based on qualifications and experience)
