Job summary

Medical University of South Carolina is seeking a Social Worker (MSW) I to support discharge planning and transition of care. Under supervision, you will assess patients and caregivers, coordinate with the health care team, and identify post-discharge risks to prevent negative outcomes.

You'll work 40 hours weekly in a hospital setting, with a $2,500 sign-on bonus. Master’s in Social Work and active MSW licensure in SC are required; Case Management certification is preferred.

Qualifications

  • Master's degree in Social Work from a CSWE-accredited program.
  • Current and active MSW licensure issued by the SC Board of Social Work Examiners is required at time of hire.
  • Certification in Case Management is preferred.

Under guidance of supervisor, the Social Worker I (MSW) assists with coordination of the discharge process to ensure a successful transition in collaboration with the health care team to achieve targeted patient outcomes. This role completes an assessment on the patient and caregivers throughout the hospitalization and outpatient follow-up as needed. This position identifies risk factors that the patient may experience after discharge and applies discharge interventions to lower the patient risk of an undesirable event post discharge. The Social Worker (MSW) bridges the discharge to outpatient provider with the patient and caregiver to address issues and questions.
