Medical University Hospital Authority (MUHA) in Charleston is seeking a Social Worker I (MSW) to support discharge planning in collaboration with the health care team. Assists patients with environmental difficulties that interfere with obtaining maximum benefits from medical care. Serves as liaison between medical and nursing staffs, patients, relatives and appropriate outside agencies. Interprets and assists in resolving social problems that relate to medical condition and/or hospitalization.
Education: Master's degree in Social Work from a program accredited by the Council on Social Work Education (CSWE).
Licensure: Current and active MSW licensure issued by the South Carolina Board of Social Work Examiners is required at the time of hire.
Preferred Qualifications: Certification in Case Management is preferred.
