Job summary

Longwood Hospice is seeking a compassionate Per Diem Hospice Social Worker to join our team. The role provides psychosocial support, counseling, and resource coordination across Eastern Massachusetts.

As part of an interdisciplinary team, you will deliver education, assess patients and families, and travel to locations as needed to support end-of-life care.

Qualifications

• BSW required; MSW preferred.
• LICSW required, or supervision under LICSW.
• Minimum one year of independent patient work experience.
• Strong communication, counseling, and organizational skills.

Responsibilities

• Provide emotional support and counseling to hospice patients and families.
• Complete psychosocial assessments and develop care recommendations.
• Assess patient and family needs, coping abilities, and resources.
• Assist with financial and community resources as needed.
• Participate in IDT meetings and collaborate with staff.

Job description

Longwood Hospice are seeking a compassionate and experienced Per Diem Hospice Social Worker to join our hospice team. This is a flexible opportunity for a social worker who is passionate about supporting patients and families through end-of-life care in skilled nursing facilities across Eastern Massachusetts.

The Hospice Social Worker provides psychosocial support, counseling, education, and resource coordination to hospice patients and their families. Working as part of our interdisciplinary team, you will help patients and caregivers navigate the emotional, social, and practical challenges associated with serious illness and end-of-life care.

Responsibilities

• Provide emotional support and counseling to hospice patients and their families.

• Complete psychosocial assessments and develop recommendations for individualized plans of care.

• Assess patient and family needs, coping abilities, support systems, and available resources.

• Assist patients and families with financial, community, and other supportive resources as needed.

• Participate in interdisciplinary team (IDT) meetings and collaborate with nurses, physicians, facility staff, and other members of the hospice team.

• Assess caregiver needs and identify opportunities for respite and additional support.

• Maintain timely and accurate clinical documentation.

• Provide education and support to patients, families, facility partners, hospice staff, and volunteers.

• Advocate for patient rights, preferences, dignity, and quality of life.

• Travel to patient locations as needed to provide care and support.

Qualifications

• Bachelor's degree in Social Work (BSW) required; MSW preferred.

• LICSW required, or must work under the supervision of an LICSW.

• Minimum of one year of experience working independently with patients.

• Strong communication, counseling, organizational, and interpersonal skills.

• Comfortable working independently with minimal supervision.

• Basic computer proficiency and ability to complete electronic clinical documentation.

• Ability to travel to patient locations as required.

Preferred Experience

• Previous hospice or palliative care experience.

• Experience working with geriatric, oncology, or seriously ill patients.

• Experience working with patients and families in skilled nursing or other healthcare settings.
