Part‑Time Hospice Social Worker (MSW)

Join our dedicated team! We are seeking a compassionate, skilled, and mission‑driven Social Worker (MSW) to join our small, dedicated hospice team. This part‑time position offers a unique opportunity to provide meaningful support to patients and families during one of life’s most important journeys while working alongside a close‑knit interdisciplinary team that values collaboration, respect, and individualized care.

About the Role

As a Hospice Social Worker, you will play an essential role in supporting patients and their loved ones through the emotional, social, and practical aspects of end‑of‑life care. You will provide counseling, advocacy, resource coordination, and guidance while helping patients and families find comfort, meaning, and support during a challenging time.

Key Responsibilities
  • Conduct psychosocial assessments of patients and families
  • Provide emotional support, counseling, bereavement and crisis intervention as needed
  • Assist patients and caregivers with advance care planning and end‑of‑life decision‑making
  • Connect families with community resources and supportive services
  • Facilitate discussions regarding patient goals, family dynamics, coping, and bereavement needs
  • Collaborate closely with nurses, physicians, chaplains, aides, and other members of the hospice interdisciplinary team
  • Advocate for patient and family needs while promoting dignity, comfort, and quality of life
  • Complete timely and accurate documentation in accordance with hospice standards

Qualifications

Masters Degree in Social Work from a CSWE accredited program. Licensed as a Social Worker through the Massachusetts Board of Social Worker. Three years recent (within the past five years) experience in acute, long‑term care or home care/hospice, with at least one in hospice or homecare preferred. Knowledge of community resources including Medicare and Medicaid, financial aid, county, state, and private social services agencies.
