As a Hospice Social Worker, you will play an essential role in supporting patients and their loved ones through the emotional, social, and practical aspects of end‑of‑life care. You will provide counseling, advocacy, resource coordination, and guidance while helping patients and families find comfort, meaning, and support during a challenging time.
Executive Director
Masters Degree in Social Work from a CSWE accredited program. Licensed as a Social Worker through the Massachusetts Board of Social Worker. Three years recent (within the past five years) experience in acute, long‑term care or home care/hospice, with at least one in hospice or homecare preferred. Knowledge of community resources including Medicare and Medicaid, financial aid, county, state, and private social services agencies.
