NHS Jobs in Lincolnshire seeks an experienced Social Worker Level 1 to deliver statutory social care support to adults with varying needs, aiming to promote independence and safety. You will assess, plan, implement and review care packages, and help people build support networks to achieve their outcomes.

You will manage a caseload with supervision, work with partner health and care services, and contribute to service development. Travel outside the county may be required to meet client needs.

Responsabilités

• Manage a caseload with supervision and advice from Level 2 Social Worker or Advanced Practitioner.
• Undertake needs assessments under the Care Act 2014 to identify care and support needs.
• Collaborate with Safeguarding colleagues to contribute to safeguarding inquiries.
• Identify non-eligible social care needs and advise on wider solutions.
• Create value-for-money care packages with individuals and carers.
• Engage with individuals and communities to support informed choices.
• Provide support through informal or community resources.
• Research resources and report gaps in service provision.
• Operate within delegated budget and associated mechanisms.
• Maintain case notes, write reports, and give evidence in court if required.
• Follow Lincolnshire County Council Adult Care policies and safeguarding procedures.
• Contribute to area-specific work arising from organisational need.
