Job Summary: The Client Advocate serves the community by building trusting relationships through the Care Mobile. Responsibilities include providing education, conducting screenings, and linking clients to resources, thereby increasing healthcare access.

Essential Functions and Responsibilities:

  • Provide resources to patients and families to promote productive lifestyles
  • Report abuse or neglect in accordance with state laws, with proper documentation
  • Offer crisis intervention and resource support, documenting in medical records
  • Work as part of a multidisciplinary team to deliver comprehensive, trauma-informed, client-centered services
  • Collaborate with the Ronald McDonald House Charities for grants, funding, and quality metrics
  • Coordinate with Social Services to address community needs and update resources related to maternal-child health
  • Support cultural responsiveness and connect clients to appropriate services
  • Manage client support services, including care plans, referrals, and community linkages
  • Assist patients with Medicaid applications
  • Perform other duties as assigned

Education and Experience:

  • Bachelor’s degree in social work from an accredited program
  • Certification or licensure as a Social Worker (or eligibility to test within one year)
  • Experience in healthcare, crisis intervention, or social services
  • Experience working with community members and coordinating with hospitals and partner agencies
