Social Worker – BSW Home Health

Sentara Health is seeking a qualified individual to join our team in South Boston, VA for the role of Social Worker BSW – Home Health.

Position Details

• Location: Sentara Hospice Services – South Boston, VA (travel may include up to a 50-mile radius)
• Shift: First (Days) – 8 AM to 5 PM
• Employment: Part‑time, 24 hours per week
• Salary: $25.60–$42.67 per hour

Responsibilities

The Social Worker will perform a variety of casework and provide social work services to patients, families, and significant others. Responsibilities include:

• Develop and monitor multidisciplinary collaboration of services to patients where appropriate.
• Assist adult patients and their families with personal and environmental difficulties associated with medical conditions, including support during the terminal illness stage.

Qualifications

• Bachelor’s Level Degree in Social Work (required)
• Valid driver’s license (required)
• Basic Life Support certification within 90 days of hire (required)
• One year of social work experience (required)
• Previous Home Care/Hospice experience (preferred)
