Job summary

VHS Hospice is seeking a Social Worker (MSW required) to assess psychosocial needs of residents, develop care plans, and coordinate services to promote well-being. Salary ranges from $62,000 to $75,000 annually, with opportunities to support residents across multiple Virginia-area facilities.

The role emphasizes empathetic counseling, collaboration with interdisciplinary teams, and adherence to facility policies, regulations, and documentation standards.

Qualifications

• Bachelor’s degree required; MSW degree required.
• Minimum of 1+ years of experience as a Social Worker in a healthcare-related setting.
• Strong knowledge of regulations, policies, and guidelines governing nursing care facilities.
• Excellent communication and interpersonal skills with residents, families, staff, and external agencies.

Responsibilities

• Address resident concerns, goals, and emotional needs while maintaining ongoing communication with residents, families, and case managers.
• Build and maintain positive, supportive relationships with residents and their families.
• Coordinate care and collaborate with interdisciplinary teams, including physical therapy and other clinical departments.
• Develop, implement, and update individualized psychosocial care plans for residents.
• Participate in care planning meetings and support continuity of care throughout the resident’s stay.
• Provide counseling, emotional support, and resource referrals as needed.
• Ensure compliance with all applicable facility policies, regulations, and documentation standards.
