Responsibilities

The Medical Social Worker in hospice provides medical social services under the direction of a physician and Interdisciplinary Group to assist with understanding social and emotional factors related to the patient's health and developing coping mechanisms.

• Assist patients, significant others, physicians, and healthcare team members in understanding personal, emotional, environmental, and social factors affecting health.
• Contribute to the development of a comprehensive, integrated Plan of Care for patients.
• Instruct healthcare team members on community resources available to assist patients.
• If needed, function as Bereavement Coordinator, supervising bereavement services and establishing a plan of care addressing bereavement needs up to 13 months after the patient’s death.

Education and Experience

License Requirements

• Bachelor's Degree in social work from an accredited school or in psychology, sociology, or related field with supervised MSW experience.
• One year of social work experience in a healthcare setting.
• Current CPR certification.
• Valid Driver's License, vehicle insurance, and access to reliable transportation.

Additional State Specific Requirements

• Bachelor's degree from an accredited School of Social Work.
