Performs duties relevant to addressing the psychosocial concerns of Memorial Health System patients and families. Mobilize health, psychosocial, and economic resources to promote optimal outcomes. Collaborates with patients and providers as an integral care team member, advocating between patients, families, healthcare providers, services, and community agencies to expedite healthcare services across the continuum of care. Education & Qualifications

• Master’s degree in Social Work from an accredited School of Social Work.

• Illinois Licensed Social Worker required within six months of hire.

Experience

• Experience working with adults with chronic or serious illnesses.
• Experience coordinating needs of chronically ill patients and supporting the care team.
• Understanding of psychosocial implications of illness, hospice, and end-of-life issues.
• Knowledge of local community resources and human behavior as it relates to health and culture.

Responsibilities

• Conduct biopsychosocial assessments within 3 days of admission, including regulatory mandated information.
• Serve as liaison between patients, families, and community agencies; coordinate information and referrals.
• Develop individualized care plans, assist with medication access, and facilitate enrollment in assistance programs.
• Collaborate with clinical teams, assist with scheduling, and arrange transportation and interpreters.
• Coordinate care transitions, maintain documentation, and build relationships with healthcare providers and community resources.
• Provide crisis intervention, supportive counseling, and advocate for patients' needs.
• Support advance care planning, including legal and medical directives.
• Provide group, individual, and family counseling as needed.
• Initiate discharge planning, ensure proper communication, and document activities thoroughly.
• Participate in quality improvement, staff training, and community education.
• Adhere to ethical standards, hospital policies, and professional growth requirements.
