Freeman Home Care, part of Freeman Health System, seeks a dedicated Social Worker to assess and plan patient care in the home care setting, coordinating with physicians and other disciplines to meet patient needs.

You will work closely with patients and families to ensure smooth discharge and appropriate community resource access.

Qualifications

• Bachelor's degree in Social Work required.
• Current Kansas Social Worker License required.
• 1 year Social Work experience in a Health Care setting preferred.

What You’ll Do

Responsible for assessment and planning patient care in conjunction with other disciplines in home care. Provides social work service up to and including discharge for patients requiring those services. Assists client in obtaining available community, State or Federal services.

Requirements

Bachelor's degree in Social Work. Current Kansas Social Worker License Minimum 1 year Social Work experience in a Health Care setting. Auto liability insurance. Clean driving record. Current driver's license. Must be able to successfully pass KDADS criminal record check upon hire. Must register with and successfully pass the Family Care Safety Registry upon hire. Preferred Requirements

Current Clinical Social Worker licensure in state of practice.
