Job summary

Freeman Health System's Freeman Home Care is seeking a dedicated Social Worker to assess and plan patient care in collaboration with a multidisciplinary home health team. You will provide social work services through discharge, helping patients access community and government resources.

The role requires a Bachelor's in Social Work, a Kansas Social Worker License, and at least 1 year of health care Social Work experience.

Qualifications

• Bachelor's degree in Social Work required.
• Current Kansas Social Worker License required.
• Minimum 1 year Social Work experience in a Health Care setting preferred.
• Auto liability insurance required.
• Clean driving record required.
• Must pass KDADS criminal record check and register with Family Care Safety Registry.

Responsibilities

• Assess and plan patient care in collaboration with other disciplines in home care.
• Provide social work services through discharge planning.
• Assist clients in obtaining community, state, or federal services.
