Job summary

Freeman Home Care in Missouri is seeking a licensed Social Worker to assess and plan patient care in collaboration with the home care team. You will coordinate with physicians, nurses and therapists to ensure comprehensive care goals are met in the home setting.

The role requires a Bachelor's degree in Social Work, an active Kansas Social Worker license, and at least one year of healthcare social work experience. Auto liability and a clean driving record are required.

Qualifications

• Minimum 1 year Social Work experience in a Health Care setting.
