Job summary

AuthoraCare Collective seeks a Social Worker Home Care in the Burlington service area. This full-time role runs Monday through Friday, 8:00am–5:00pm, supporting hospice patients and their families with comprehensive psychosocial assessments, counseling, and care planning.

The ideal candidate has a Master’s degree in social work and is licensed or in the process of licensure (LCSW/LCSWA) with at least one year of related experience in a mental health or medical setting.

Qualifications

• Master's degree in social work is required.
• LCSW OR LCSWA (or in the process of obtaining) is required.
• One year of social work experience in a mental health or medical setting is required.
• Valid state-issued driver’s license; automobile liability insurance; must have own transportation.

Responsibilities

• Prepares psychosocial assessments.
• Develops individualized plans of care related to grief, adjustment, care, and end-of-life planning for patients, families, and caregivers.
• Identifies appropriate resources to meet patient/caregiver/family needs.
• Acts as an advocate for patients and families.
• Collaborates with the interdisciplinary group to coordinate care and report to the IDG.
• Completes required documentation per licensure and agency standards.

AuthoraCare Collective is currently seeking a Social Worker Home Care supporting our Burlington service area. This is a full-time position; Monday - Friday 8:00am - 5:00pm.

Job Summary

Responsible for providing comprehensive assessment of the social, emotional and practical needs of the Hospice patients and their support systems. Provides advanced counseling, education and referrals to address patient/family issues of grief and loss, adjustment, care needs and end of life planning. Provides educational programs to the community. Functions as a member of an interdisciplinary team

of care.

Requirements

• Master's degree in social work

Delivery of Care

• Prepares psychosocial assessments.
• Develops and carries out individualized plans of care related to the grief and loss, adjustment, care, and end of life planning needs of patients, families, and caregivers.
• Identifies appropriate resources to meet patient/caregiver/family needs.
• Serves as an advocate for patients and families.

Interdisciplinary Group Process

• Actively participates with the interdisciplinary group (IDG).
• Works effectively with other IDG members with a focus on collaboration in problem-solving patient needs and working with the patients’ plan of care.
• Provides effective report to the IDG based on regulatory requirements for patient care planning evaluation.
• Maintains effective relationships and partnerships with community healthcare providers, agencies, and faith communities.

Documentation

• Completes required documentation according to licensure and certification requirements and agency standards including accreditation standards.
• Ensures that notes and patient records are complete, accurate, current, and clear.
