WelbeHealth, LLC. seeks a Social Worker to manage a caseload of frail seniors in collaboration with an interdisciplinary team. You will conduct psychosocial assessments, develop care plans, provide counseling, and coordinate with families and care teams to ensure comprehensive support.
Responsibilities include documentation of interventions, facilitating care transitions, and discharge planning. CPR/First Aid certification within 180 days is expected, with MSW education and elder-care experience
At WelbeHealth, each participant of our program is guided by our interdisciplinary team (IDT) - composed of clinical and non-clinical members who work cohesively to provide quality comprehensive primary care and social services for frail seniors to keep them living safely in the community. Reporting to the Social Work Manager, the Social Worker maintains a caseload of participants working in close coordination with the IDT. In addition to delivering standard social work services such as psychosocial assessments, care planning, counseling and case management, the Social Worker functions as a liaison between participants, family members, support networks, and care teams as appropriate.

Compensation consists of base salary plus bonus. WelbeHealth offers a competitive total rewards package that includes a 401(k) match, comprehensive healthcare coverage, and a broad range of additional benefits. Actual compensation will be determined based on experience and relevant qualifications.
