Avon Healthcare in Adrian, MI is hiring a full-time Social Worker to support two closely partnered buildings located under 2 miles apart. This shared role provides day-to-day support to the Social Services Director across both Adrian Bay and Springcreek facilities.

You will collaborate with residents, families, and the care team to ensure smooth admissions, planning, and advocacy, while maintaining regulatory documentation and promoting residents’ emotional well-being.

Qualifications

• Active Social Work state licensure preferred.
• Experience in healthcare, skilled nursing, or long-term care required.
• Strong communication, organization, and advocacy skills.

Responsibilities

• Provide psychosocial support to residents and families.
• Assist with admissions, care planning, and discharge planning.
• Coordinate with interdisciplinary teams to support resident needs.
• Support residents through transitions and changes in care.
• Ensure documentation meets regulatory and facility standards.
• Advocate for residents’ emotional and social well-being.
