Owensboro Health Regional Hospital in Owensboro, KY, is seeking an RN Case Manager to facilitate cost-effective, high-quality patient transitions of care. You will collaborate with staff and provide guidance, education, and support to assigned patients throughout the discharge process.

Responsibilities include coordinating discharge planning, performing comprehensive assessments, educating patients and families about post-hospital care options, and facilitating referrals to community resources.

Qualifications

  • RN licensure in Kentucky at hire
  • Bachelor's degree in Nursing required upon hire
  • Ability to perform comprehensive assessments and discharge planning
  • Strong communication with patients, families, and healthcare teams
  • Ability to train new staff and students as needed

Responsibilities

  • Coordinates and manages discharge planning activities.
  • Completes a comprehensive assessment of medical, financial, emotional status and support systems to develop discharge plans for post-hospital care.
  • Collaborates with patients, families, and healthcare team members including physicians and nurses.
  • Provides information and education on post-hospital care options and resources.
  • Facilitates referrals to external facilities and community services.
  • Ensures paperwork and documentation are completed accurately and timely.
  • Monitors progress and adjusts discharge plans as needed.
  • Communicates with management on complex discharge issues and delays in service.
  • Trains new employees, students, or interns as needed.

Weekend Requirements: Every | Work Hours: 0700-1930 | Status: Per Diem/PRN - 0.0 FTE (less than 40 hours per pay period) | Entity: Owensboro Health Regional Hospital | Department: Case ManagementJob Summary:Facilitates cost-effective, high quality patient transitions of care and outcomes by collaborating with staff and providing guidance, education, and support to assigned patients.Job Responsibilities:Coordinates and manages discharge planning activities.Completes a comprehensive assessment of the patient's overall medical condition, financial status, emotional status, and support system to develop a discharge plan for post hospital care.Collaborates with patients, families, and healthcare team members, including physicians and nurses.Provides patients and families with information and education on post hospital care options and available resources.Facilitates patient referrals to external healthcare facilities and/or other community servicesEnsures all necessary paperwork and documentation are completed accurately and timelyMonitors patient progress and modifies the discharge plans as necessaryCommunicates with Manager, Director, and/or Physician Advisor on complex discharge planning issues and delays in service.Trains new employees, students, or interns as needed.Skills and Attributes:Requires critical thinking skills and decisive judgment.Works under minimal supervision.Must be able to work in a stressful environment and take appropriate action.Position Requirements:Work ExperienceA minimum of 1 year relevant experience preferred.EducationBachelor's degree or higher in Nursing required upon hire.Licensure/Certification/RegistrationRN - Licensed as a Registered Nurse.
